If you know others who feel this person should also be inducted, encourage them to send in a nomination form as well.

All forms must be received by Nov 30th of the current year to be considered for the following year's inductions.

RE-NOMINATION
If someone has been nominated more than 5 years ago, they must be re-nominated in order to be kept for consideration to be inducted into the Hall of Fame.  You will need to re-nominated them with the form below.
